DMHC Autism Program

Description of Services Provided:
The Center offers autism spectrum disorder diagnostic and treatment services for children through 18 years of age.  Treatment options include: occupational therapy, speech and language pathology, family outreach services through its P.L.A.Y. Project  (Play and Language for Autistic Youngsters), sensory integration therapy services, and Applied Behavioral Analysis.

Elk, Cameron, McKean, Potter, Jefferson, and Clearfield Counties

Admission Criteria:
Needed services are determined after a face-to-face appointment with a clinician to address presenting problem areas, as well as past personal history, after which a complete autism evaluation will be scheduled if deemed necessary.

How to Access Services:
Services are most commonly accessed through phone contact with the office, a referral from a family physician, or referral from Early Intervention or the schools.

Description of Service provided:
Peer Specialist services are specialized therapeutic interactions conducted by self-identified current or former consumers of mental health services. The peer specialist is trained and certified to offer support and assistance in helping others in their recovery and community-integration process. Peer support is intended to inspire hope in individuals that recovery is not only possible, but also realistically achievable.

The program philosophy is that all peer specialists are examples of hope and can enlighten people as to what recovery is and can teach the skills to help facilitate it. Recovery pathways give people direction and assistance in achieving individualized goals, while recovery principles and wellness tools provide ways to maintain wellness and divert possible problems. Services are based on mutuality and respect. In peer encounters mutuality provides bonding, role modeling, friendship, companionship and a common ground.

Consumer choice is crucial and the individuals participating in the program are encouraged to take an active role in their goal planning and recovery process. Services can include but are not limited to:

This is a voluntary program. The individual must be 18 years of age or older, with a Serious Mental Illness as defined by the Mental Health Bulletin number OMH-94-04.

Peer Specialist referrals can come from a variety of different sources including: psychiatrists, outpatient therapist, Partial Hospitalization Program, case managers, psychiatric hospitals, other service providers involved in their current treatment and self-referrals.

Photo of CrossroadsChild & Adolescent Partial Hospitalization Program

Description of service provided:
This is a structured day-treatment program for children and adolescents who have a mental health diagnosis.  We provide group, individual, and family therapy.  Clients also receive medication management through our Board Certified psychiatrist.  Group therapy focuses on skills building and learning more about symptoms, moods, and coping strategies.  During the school year, clients are still enrolled in their home schools.  The classroom subjects and Coudersport Annex Main Entranceassignments are the same as the classroom subjects and assignments at the client's home school.  In the summer, we continue to have therapy groups, which include recreational activity.  We do not have school during the summer.  Some of the therapeutic issues we address to help our clients are: anger management, social interaction, and self-esteem.  The effectiveness of the program is measured by consumer Treatment Plans with individualized goals and objectives.

Admission Criteria:
(A) Between the ages 12 and 18, (B) Have mental health diagnosis, (C) Display behaviors which indicate a risk of safety for self or others, and (3) Be recommended as medically necessary by a family doctor, psychologist, or psychiatrist.  Other admission criteria focus on specific symptoms and their level of severity, parent-child involvement with treatment, and documentation that the child's mental health needs cannot be met with appropriate supports in the school setting.  Call for detailed information.

Description of service provided:
The adult partial hospitalization program offers a wide range of therapeutic activities, including group and individual therapy, to adults and adolescents with moderate to severe mental health problems who require a more structured and intensive therapeutic experience than can be provided at an outpatient clinic, but is less restrictive than inpatient care.  Adult partial hospitalization program provides various groups throughout the day.  These groups address mental health issues such as diagnoses, symptoms, medications, feelings, thoughts, social skills, relationships, and coping skills.  Participants are scheduled to meet with their primary caseworker on a regular basis to review goals from their treatment plans & discuss other issues they may have.  Psychiatric services Coudersport Annex Main Entranceconsisting of medication evaluation and management, psychiatric assessments, monthly follow-up sessions, and determination of medical necessity are available.

This behavioral health care service is delivered in the most accessible and effective manner possible.  The goal of the program is to stabilize a person's mental health symptoms to assist them in returning to their previous roles in the community.

Description of Service Provided:
The Recovery Program works with mental health consumers who meet the criteria of Serious Mental Illness and are 18 years or older. Special authorization exception is needed if the individual is under 18 years of age. This program is built on the Key Recovery Concepts of: Hope, Personal Responsibility, Education, Self-Advocacy and Support. The course of recovery is individualized and it is important to understand that the consumers are ultimately the experts in their own recovery and must take an active role in their own goal planning. The ultimate goal of the program is to develop skills necessary for living in the community, so that one's illness is not the driving force in one's life. Specific goals that have been addressed include but are not limited to:

· • Assistance developing and working towards recovery goals
  • Activities of daily living
  • Assistance accessing housing, income support, medical, social/recreational, spirituality, educational and vocational services
  • Mental health support and community outreach
  • Building social supports
  • Reducing relapses and increasing coping skills
  • Symptom management

Coudersport Annex Main EntranceFamily Based Services

Description of service provided:
Family Based Services in an intensive, non-traditional therapeutic program that meets the needs of the family system.    Services are provided for 32 weeks (an 8-month) treatment period, which can be lengthened or shortened depending on the needs of the family.  The main goals of family Based Services is to enable parents to care for their children at home, to strengthen and maintain the family, improve coping skills, teach positive parenting, provide the family with case management, and to act as an advocate for the child. 

 

As part of this program, a 24-hour crisis hotline is available to families 7 days a week in the event of a family crisis.  Additionally, Family Based Services staff work non-traditional hours, which include evenings, and occasionally weekends if necessary in order to meet the needs of the family. 

 

The Family Based team is made up of a master’s level and a bachelor’s level mental health professional.  This team provides services directly in the home, community, and school helping the family to identify their strengths, and assisting them in meeting their needs.  Contact generally consists of one family session per week, one parent session per week, and an individual session with the identified child per week.  However, if needed additional session can occur with other family members as necessary.

Admission Criteria:
Children and adolescents can be referred by a number of sources including family members, schools, primary care physicians, case managers, an outpatient therapist, the CASSP program, an inpatient hospital stay, etc.  The referred child needs to meet criteria for a mental health diagnosis and Family Based Services must be authorized by a psychologist or a psychiatrist who feels this level of care is appropriate for the child and their family.

Employment Supports Services is a program offered through Dickinson Mental Health Center and has been serving Cameron, Potter, and Elk Counties for over seventeen years.

Description of services provided
Dickinson Mental Health Center's C.O.P.E (Coalition of People Empowered) Drop in Center is a center for socialization and recreation for individuals of mental health services in Elk and Cameron Counties. The center has many activities to offer including but not limited to: pool table, TV, DVD Player, a library, kitchen, computer and craft area, membership meetings, wellness and exercise groups, and WRAP (Wellness Recovery Action Plan). Also, consumers may attend concerts, amusements parks, picnics and various recreational events based upon individuals' interest and the availability of funding.

The C.O.P.E. Drop in Center admission criteria includes individual that are over the age of eight-teen and have a mental health diagnosis.

How to access service:
The C.O.P.E. Drop in Center serves all individuals that fit the above criteria. The Drop in Center does not accept referrals from care providers. If an individual is interested in coming all they need to do it just call 776-2664.

Description of Services Provided and Accessing Care:
Department of Public Welfare licensed Crisis Services are available between the hours of 8:00 a.m. through 5:00 p.m. Monday through Thursday and 8:00 a.m. through 4:00 p.m. on Fridays in Cameron, Elk, and Potter Counties.  For services in Cameron and Elk County call 814-776-2145 in Potter County call 814-274-8729.  Services are
available to any individual experiencing an acute disturbance of mood, thought,
Coudersport Annex Main Entrancebehavior and/or social functioning. Services are provided by competent mental health professionals who are trained and qualified.

A telephone screening will be completed with every consumer or concerned party seeking crisis intervention services.  A solution focused supportive counseling approach will be used when conversing with consumers experiencing a crisis.  In the event the consumer requires or is requesting a face to face crisis assessment, this will be provided on a walk-in or mobile outreach basis. 

Upon completion of a crisis evaluation, the crisis worker and consumer will review and consider treatment options which will initiate resolution of the problems identified by the consumer. Each consumer will be referred to the most appropriate and least restrictive level of care based on medical necessity criteria.  Treatment alternatives that are most often considered include; outpatient therapy, specialized community based and children’s services or inpatient psychiatric care and stabilization.

Services in Cameron County are limited to telephonic crisis intervention, however, its consumers may walk-in at either the Elk or Potter County location.  Provisions for emergency care will be arranged.

When the need for an involuntary psychiatric examination is clinically necessary under Section 302 of the Mental Health Procedures Act of 1976, a Mental Health Delegate serving the county where the emergency is taking place will be contacted to assist with arranging emergency care.  A psychiatric emergency may be defined as any instance in which an individual poses a  serious danger to self or others and a dangerous act has occurred or is imminent.

Dickinson Mental Health Center does not provide crisis or emergency intervention services outside of its posted hours of licensed operation.  After  hours and during holidays and weekends, a Mental Health Delegate will evaluate the potential need for emergency involuntary psychiatric examination.  In Elk and Cameron County a Delegate is available by calling 1-800-652-0562. In Potter County, call 911 and request the Delegate on call.

Consumers seeking voluntary inpatient psychiatric care after hours are encouraged to travel to the closest facility offering this level of care to request an evaluation.  If circumstances are such that the consumer is uncertain of the potential need for voluntary inpatient care and the situation is safe, consumers may request a crisis assessment during the posted hours of operation.

There are no age restrictions for service eligibility.  The principle criteria for admission include imminent risk of self harm or harm to others; and any disturbance in mood, thought, behavior, or social functioning.

Description of service provided:
Family Group Decision Making is a voluntary program to find solutions for family problems. The planning is unique because all plans are developed by the family themselves, extended family and community members that the family invites to a planning conference. The referral source and service providers then act as consultants to the family to come up up with a good working plan. After the plan is determined the referral source and service providers act as supports to the family to assist them in ensuring the plan's success.

Description of service provided:
Forensics consists of three arms of services all related to the criminal justice/mental health population.  The first arm consists of services provided to the Court and include assessments provided for both competency and sentencing purposes.  The second arm consists of in jail services which include assessment, therapy psychiatric intervention and lethality assessments.  The final arm of the Forensics program is the Outreach section which follows inmates from both the County and State prisons back into the community to ensure successful re-integration through service coordination, therapy, and psychiatric services.

Clients must have a diagnostically supported Axis I diagnosis and be currently involved in the criminal justice system through incarceration and/or probation and parole.

Coudersport Annex Main EntranceOutpatient Mental Health Counseling

Description of service provided:
Using a variety of treatment models and techniques, counseling/therapy is provided to individuals, couples, families and groups of all ages to alleviate the symptoms of mental illness or restore previous levels of functioning.  Treatment options include; counseling/therapy, medication monitoring and management through a psychiatric nurse and psychiatrist, and/or crisis services.

Admission Criteria:
Needed services are determined after a face-to-face appointment with a clinician to address presenting problem areas, as well as past personal history.

How to Access Service:
Services are most commonly accessed through phone contact with the office, a referral from a family physician, or referral from other social service providers.  Crisis Services, for emergent care needs, can be accessed Monday - Friday 8am - 5pm.

Case Management

Description of service provided:
Resource Coordination (RC) and Intensive Case Management (ICM) services assist seriously and persistently mentally ill people in gaining access, coordinating and monitoring necessary mental health, medical, social, educational and other eligible services to successfully remain in the community.  Consumers must satisfy specific mental health criteria established by the Commonwealth to participate.

Resource Coordination: Frequency of contact is determined by need, but at minimum, should include every 30 days for children and every 60 days for adults. 

Intensive Case Management: Features include contact by the ICM at least every 14 days with a 24-hour availability through the crisis hotline.

Outpatient Waiting Room - Main Office - RidgwayChildren's Outpatient Services

Description of service provided:
Children present for outpatient services at DMHC for a wide range of mental health concerns (e.g. ADHD, adjustment issues, depression, anxiety, etc.)  Therapy with children is typically different than therapy experienced by adult clients.  Younger children may engage in play therapy (e.g. playing with toys or games) or art therapy (e.g., painting or drawing) during sessions.  Older children and adolescents may engage in more traditional talk therapy to address their mental health issues.  When serious underlying emotional problems are determined, the child may be referred to a psychiatrist for consideration of medications to support the child with his/her recovery. 

DMHC has launched a School Based outpatient program in Elk County schools effective September 2008.  H.O.P.E. (Helping Our Pupils Excel) will provide for ready access to mental health treatment in a school-based setting. This service will be able to reach high school students that may not be able to access services in a traditional OP setting.  Parental involvement is strongly encouraged and supported by the program.  Similar program services are already operational in Potter County's school classrooms.Photo of Emporium Office

Admission Criteria:
Children and adolescents can be referred by a number of sources including family members, schoools, primary care physicians, case managers, etc.  Requests for children's outpatient services are evaluated by a mental health professional who assists in determining the appropriateness of outpatient services.  The referred child needs to meet criteria for a mental health diagnosis to be able to be seen in the outpatient setting.

Coudersport Main OfficeInterested individuals can call (814) 776-2145 in Elk County, and (814) 486-0554 in Cameron County to talk with a mental health professional about scheduling an intake session.  The Elk County Outpatient clinic is located in Ridgway while the Cameron County clinic is located in Emporium.  For school based services in Potter County (Coudersport) please contact (814) 271-8651 for more information.

Clients can pay for outpatient services by Medicaid, private insurance, or self pay.  If a family does not have insurance coverage, a sliding fee scale can be utilized to determine payment for services based upon the family's income level.*
