Intensive Case management. Intensive Case management is a voluntary program designed to help both adults and children with long term emotional problems find the resources and to provide the support they need to avoid being hospitalized and improve the quality of their lives. It targets the persistently and the chronically mentally ill. As a part of this program, a 24 hour crisis hotline is available to all consumers in an effort to offer timely intervention when needed, as needed. Additionally, Intensive Case managers do not work nine to five every day; rather, they work a flexible schedule that often includes evenings and weekends. The Intensive Case manager is not a therapist; but, rather, just as the name implies, someone who manages a case on an intensive basis. Contacts with consumers are usually at least weekly, and can never be less than every fourteen days. Resource Coordination Resource Coordination services are available in Elk and Cameron Counties to adults with serious and persistent mental illness and children and adolescents with or at risk for serious mental illness who do not need the intensity or frequency of contacts provided through Intensive Case Management. Resource Coordination services assist eligible persons in accessing needed resources such as medical, social, educational and other services. The Resource Coordinator carries a higher caseload census than the Intensive Case Managers but there is no 24 hour availability requirement as there is with ICM. Supported Living This program is an extension of typical Case management servicing MR individuals residing independently in the community. Case managers focus on locating housing and providing training with independent living skills such as budgeting or grocery shopping as well as enhancing natural supports within the community. Individuals with mental retardation or mental health consumers that may require extensive training in daily living skills to maintain independence may utilize M.R. Supported Living Skills or M.H. Housing Support Services. These Programs can provide several skills training that is directed and monitored by Case management.
