AHC Hosts Food Drive

The individuals who attend the Adult Habilitation Center, a program of Dickinson Center, Inc., recently hosted a food drive to help supplement the Good Samaritan Food Bank for the holidays. The AHC is a licensed center-based day program for adults with intellectual and developmental disabilities and for adults over 60 years of age with functional impairments. Money, collected at Dickinson’s annual employee picnic, was also donated along with the food items. Additional donations to the AHC food drive were collected during a recent joint advisory council Christmas party hosted by the Elk, Cameron and McKean County RSVP. The RSVP is sponsored by the Office of Human Services, Inc. Area Agency on Aging and hosts or participates in several community outreaches throughout the year in an effort to advance volunteerism in the three counties. Reusable grocery bags, supplied by Northwest Health Connections were also given to the food bank. Description of service provided:
Direct service employees provide services to adults with developmental disabilities who live in community settings which include families, group homes and Lifesharing. Services consist of work experience and other developmental work training activities to promote movement into a higher level vocational program. Activities include: 1) training designed to teach job-related skills; 2) personal work and adjustment training designed to develop appropriate worker traits and teach an understanding of the work environment; and 3) assessments of an individual’s vocational aptitude and potential. Elkwood Arts produces various wood products which are sold through a variety of means. Individuals receive a wage based upon job completed. Some jobs pay at a sub-minimum wage level while others may be at minimum wage or higher. Money to pay wages comes directly from the sale of our products. Dickinson Center, Inc. maintains a sub-minimum wage certificate.

The day is structured to include individual and group work activities. Services must meet regulatory requirements of programs licensed under the 55 Pa. Code Chapter 2390 and fall within the Waiver Service Definitions authorized by the Office of Developmental Programs and the Center for Medicaid and Medicare Services.

Dickinson Center, Inc. has been certified as a Qualified Provider of these services by the Elk and Cameron MH/MR Program and the Office of Developmental Programs.

Geographic area of service:
Elkwood Arts service individuals that reside in Elk and Cameron Counties of Pennsylvania.

Admission Criteria:
Individuals seeking admission must have a diagnosis of mental retardation prior to the age of 18. The programs may serve anyone 18 and older.

How to Access Service:
Persons interested in services must contact the local administrative entity (AE). The AE in collaboration with Supports Coordination will work with the individual and their family in meeting financial eligibility requirements.

Coudersport Annex Main EntranceTransitional Work Services

Description of service provided:
Transitional work services consist of supporting individuals in transition to integrated, competitive employment through work that occurs in a location other than a facility subject to 55 Pa. Code Chapters 2380 or 2390. Transitional work service options include, but are not limited to, mobile work force, work station in industry, affirmative industry and enclave. A Mobile work force uses teams of individuals supervised by a training/job supervisor, who conduct service activities away from an agency or facility. The provider contracts with an outside organization or business to perform maintenance, lawn care, janitorial services, or similar tasks and the individuals are paid by the Provider.

Dickinso, Inc. operates two cleaning crews in Potter County. Each year, we initiate and negotiate contracts with local businesses to provide this service. Currently there are two businesses that access our program. The contract includes costs to cover the wages paid to the individual. Individuals are paid minimum wage. Both crews operate under the direct supervision of qualified staff.

Dickinson Center, Inc. has been certified as a Qualified Provider of these services by the Elk and Cameron MH/MR Program and the Office of Developmental Programs.

Geographic area of service:
Transitional Work crews serve individuals who reside in Potter County.

Admission Criteria:
Individuals seeking admission must have a diagnosis of mental retardation prior to the age of 18. The programs may serve anyone 18 and older.

Persons interested in services must contact the local administrative entity (AE). The AE in collaboration with Supports Coordination will work with the individual and their family in meeting financial eligibility requirements.

The program operates in local businesses and is subject to the needs of the industry. Most services are rendered in the late afternoon.

How Individuals Pay for the Service:
Applicants may be determined eligible for funding provided through the Department of Public Welfare, Office of Developmental Programs and local Administrative Entities such as Potter County Human Services.

Payment for services may be rendered through the Consolidated Waiver, Person/Family Driven Waiver, Mental Retardation Base Dollars, and Private Pay. Waiver dollars are a combination of federal and state money. Mental Retardation base money comes directly from the local County office.

This service may not be funded through either Waiver or through base allocation if it is available to individuals under a program funded under section 110 of the Rehabilitation Act of 1973 or section 602 (16) and (17) of IDEA. Documentation must be maintained in the file of each individual receiving these services to satisfy the state assurance that the service is not otherwise available under a program funded under the Rehabilitation Act as amended or IDEA.

Community Residential Programs                                                    Realistic Job Preview Community Residential Programs

Description of service provided:
Dickinson Center, Inc. operates three Community Residential Facilities, or group homes, in Elk County and one in Potter County. Direct care staff employed in these group homes provide services to adults with intellectual disabilities who choose to live in a community setting. Our goal is to encourage independent quality lifestyles while ensuring the safety, welfare and health of the individuals we serve.

Staff at the group homes provide 24-hour care and supervision for the consumers. Each home provides support to 3-4 people (Potter County home 3 people). The group homes are individualized, and every effort is made to operate the home and respond to the consumers in a manner that suits their desires and interests. Staff provide support that promotes each consumer’s independence while allowing them to accomplish everyday tasks. Each consumer’s role in self-care, cooking, laundry, and upkeep of their home is based not only on their interests, but on their capacity to participate in the task at hand.

Potter County Group HomeDuring the week, the consumers have the option to work or volunteer within their community. They may also attend day programs where they spend time with other adults developing new skills through life experiences that will enhance their self-identity. At home, the consumers are encouraged to make healthy choices and decisions while enhancing their skills and interests through personalized goals. They can often be found enjoying dinner on the back patio, painting their fingernails, playing board games, making a craft, or watching a favorite movie. Community events include shopping, eating out, picnics, dances, and local events.

Each person is encouraged to provide input into every aspect of their day in order that they may experience success and stability. Control in life, individuality, and freedom are all respected. Regular individual and group activities give each person the opportunity to learn more about themselves and their community while exploring interests and developing new skills.

The group home system operates every day of the year. Direct care staff participate in an extensive orientation period and ongoing training. Emergency relief, or substitute, staff provide additional support for special events and for staff training opportunities and time off.

Direct care staff provide assistance to the consumers with cooking meals, house cleaning, laundry, and activity planning. For some consumers, they may assist with personal care such as bathing, dressing or eating. They may also assist people in taking medications, attending medical appointments, or exercising. Many of our consumers have physical disabilities. They may need assistance transferring from a wheelchair to a bed, moving from a sitting position to a standing position, or walking on uneven ground. The training that staff receive consistently reflects the individualized supports each person requires in order to enjoy a safe, comfortable and healthy home environment.

Geographic Area of Service: The Elk County group homes are located in Ridgway, PA, while the Potter County home is in Coudersport, PA.  Any adult with any intellectual disability, regardless of their current county of residence, can be considered for referral.

Admission Criteria: Admission into the group homes is managed through the service coordination system of the Cameron/Elk Counties MH/MR Program and Potter County Human Services. Any adult with an intellectual disability can be considered for referral. Once accepted, an annual plan of service is developed with each individual, based on their interests and abilities. The plan is easily adapted to their changing needs. Supports are individualized, staff are held accountable in maintaining those supports, and families are included in the planning process.

Administrative Entities may also use monies provided by the local county to fund services. This is called Mental Retardation Base Funding. In some cases an individual may have enough private funds to pay for the service. All residents must pay rent.

Employment Support Services

Description of service provided:
Supported Employment services are direct and indirect services designed to find and support individuals in community employment work sites with coworkers who are not disabled. People in this program are paid directly by an employer and receive minimum wage or higher.

Supported employment services consist of paid employment for persons who, because of their disabilities, need intensive ongoing support to perform in a work setting. Supported employment includes activities needed to sustain paid work by individuals receiving services, including supervision and training. Photo of DMHC's Annex building

Supported Employment services consist of two components: job finding and job support. Job finding may include but is not limited to interview assistance, employer outreach and orientation, resume preparation, job searching, and preparation for job tasks. Other examples of activities that may be associated with job finding include participation in individual planning for employment development of job seeking skills: development of customer-specific job skills; employment-related personal skills instruction; job analysis; support to learn job tasks; outreach with prospective employers on behalf of the individual including consultation on tax advantages and other benefits; and consultation on behalf of an individual with the Office of Vocational Rehabilitation (OVR) benefits counseling agencies, and provider networks under Ticket to Work.

Coudersport Annex Main EntranceJob support consists of training individuals in job assignments, periodic follow-up and/or ongoing support with individuals and their employers. The service must be necessary for individuals to maintain acceptable job performance and work habits including assistance in learning new work assignments, maintaining job skills and achieving performance expectations of the employer. Other examples of activities that may be associated with job support include direct intervention with an employer, participation in individual planning for employment, support to relearn job tasks, coordination of financial issues, training to assist individuals in using transportation to and from work, maintenance of appropriate work and interpersonal behaviors on the job, follow-along services at the work site after OVR funded services are discontinued, and technical assistance and instruction for the individual’s coworkers that will enable peer support.

The Employment Support Services program primarily serves residents of Elk, Cameron, Potter and McKean Counties.

Individuals seeking admission must have a diagnosis of mental retardation prior to the age of 18. The programs may serve anyone 18 and older.

Description of service provided:
Home & Community Habilitation Services are designed to provide a variety of supportive services to Home and Community Habilitation is a direct service that consists of services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. Habilitation may be provided up to 24 hours a day to ensure the individual’s health and welfare, based upon the needs of the individual. This is a service that may be provided to people in their own home or in other residential or community settings not subject to licensing regulations.

Through the provision of this service individuals learn, maintain, or improve skills through their participation in a variety of everyday life activities. They learn and use skills in the context of these activities: this can be considered a functional approach to the delivery of services. These activities must be necessary for individuals to live in the community, to live more independently, or to be more productive and participatory in community life. Services must be provided in a manner that ensures the person’s health and welfare.

Lifesharing Through Family Living

DMHC is looking for people interested in becoming Lifesharing providers.

Description of service provided:
Lifesharing is somewhat different from other licensed homes as these settings provide for Lifesharing arrangements. People live in host homes and are encouraged to become contributing members of the family unit. Lifesharing arrangements are chosen by people and families in conjunction with host families and in accordance with the person’s needs.

The process begins with an assessment of the host home and the family. A survey is designed to take a look at the physical structure of the home as well as how the family interacts, their needs, and values. A survey is also completed with the individual. The survey is designed to get a sense of the individual’s wants, needs, and desires. Should a tentative match be found between an individual and a host home, an introduction is made? If there appears to be interest on both parties, a “getting-to-know-you” period begins. It is a combination of visits, activities, short term and long term stays at the home. The process can take as long as six months. If at any point of time, the individual expresses a desire to live with the host family and the host family agrees, a representative from the Office of Developmental Program is contacted to conduct a licensing review of the home. Once approved, the individual is free to move into the home. All host home Providers must undergo training before becoming eligible to become a Provider of Lifesharing services. Up to 30 days per year of substitute care is available to Lifesharing Providers to allow them time away from the individual to deal with personal issues.

Services assist individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. They can include help with cooking meals, house cleaning, laundry, and activity planning. They may also assist with personal care, taking medications, attending medical appointments, or exercising. These supports are designed to promote independence while allowing each person to accomplish everyday tasks in order that they may live a quality life. While residing in a Lifesharing home, individuals attend some type of day programming. Day programming varies from one individual to the other involving formal and informal activities. Services must meet regulatory requirements of homes licensed under the 55 Pa. Code Chapter 6500 and fall within the Waiver Service Definitions authorized by the Office of Developmental Programs and the Center for Medicaid and Medicare Services.

Dickinson, Inc, has been certified as a Qualified Provider of these services by the Elk and Cameron MH/MR Program and the Office of Developmental Programs.

Geographic area of service:
This program serves individuals that reside in Potter County, but can include individuals from anywhere in Pennsylvania. The program would like to expand into both Elk and Cameron Counties.

Admission Criteria:
Individuals seeking admission must have a diagnosis of mental retardation prior to the age of 18. Generally individuals are able to self-preserve meaning able to evacuate with minimal or no assistance in response to a fire alarm.

How to Access Service:
Persons interested in services must contact the local administrative entity (AE- Potter County Human Services). The AE in collaboration with Supports Coordination will work with the individual and their family in meeting financial eligibility requirements.

Adult Training Facilities and Older Adult Day Services

Potter County Adult Development Training BuildingDescription of service provided:
Direct service employees provide services to adults with developmental disabilities who live in community settings which include families, group homes and Lifesharing. Based upon the principles of normalization and Everyday Lives, individuals are presented with opportunities to gain and enhance life experiences. Services consist of supervision, training, and support in general areas of self-care, communication, community participation, and socialization. Areas of emphasis include: therapeutic activities, fine and gross motor development, mobility, personal adjustment, use of community resources, and relationship development. These activities are designed to promote independence while allowing each person to accomplish everyday tasks in order that they may live a quality life. There is a minimum of 1:6 staff ratio maintained at all times. The day is structured to include individual and group activities. Individuals are always encouraged to participate and guide the program to their greatest extent and interest. Services must meet regulatory requirements of programs licensed under the 55 Pa. Code Chapter 2380, Older 6 Pa. Code Chapter 11 Adult Day Services and fall within the Waiver Service Definitions authorized by the Office of Developmental Programs and the Center for Medicaid and Medicare Services.

Dickinson Center, Inc. has been certified as a Qualified Provider of these services by the Elk and Cameron MH/MR Program and the Office of Developmental Programs.

Geographic area of service:
The Adult Training Program primarily services individuals that reside in Elk, Cameron, McKean, and Potter Counties of Pennsylvania.

Applicants may be determined eligible for funding provided through the Department of Public Welfare, Office of Developmental Programs and local Administrative Entities such as Cameron and Elk MH/MR Program and Potter County Human Services.

Payment for services may be rendered through the Consolidated Waiver, Person/Family Driven Waiver, Mental Retardation Base Dollars, Private Pay, and Human Service Development Funds. Waiver dollars are a combination of federal and state money. Mental Retardation money comes directly from the local County office. *
